This document contains the PRIVACY POLICY of the Hospital where You are being treated (hereinafter, the “Hospital“) and is intended to detail the way in which the personal data that You provide to the Hospital and/or that are collected through the APP of the Home Medication Delivery Management Program (hereinafter, the “Program“) will be obtained, processed and protected.

 Who is the Controller of the processing of your personal data?

It is the Hospital.

What personal data does the Hospital access as a result of its relationship with you?

  • Identification data: Full name, ID card number and date of birth (age).
  • Contact and communication data: Telephone number and postal address.
  • Medical and health data: Prescribed medication and medication administrations.

Where does the data we process about you come from?

From the User: The data you provide to the Hospital at the beginning of your treatment or through any interaction you have with the Hospital.

 What does the Hospital use personal data for?

  • For the management, administration and control of your participation in the Program. The legal basis for the processing is the relationship formalized for your participation in the Program (Program Membership Agreement) and in the legitimate interest of both parties.
  • To manage the download, access and use of the Program’s APP. The legal basis for the processing is the relationship formalized for your participation in the Program (Program Adhesion Agreement) and in the legitimate interest of both parties.

 Commercial communications

The Hospital will not send commercial communications.

 With whom will your personal data be shared?  

The personal data of Hospital users will be accessible to:

  • Authorized Hospital personnel.
  • Third parties or collaborators who provide services to the Hospital in relation to the Program. In this case, such access to data on behalf of third parties will be regulated in the corresponding Contract for the Processing of Personal Data in compliance with the applicable regulations.
  • Authorities and administrations, as required by law.

 How long will the Hospital keep the personal data?

Your personal data will be kept for the time strictly necessary for the purposes of the Program for the use of which they have been provided, and, in any case, following the principle of data minimization contemplated in the applicable regulations.

As a participant in the Program, the Hospital will keep your personal data for the duration of your relationship with the Program and from the moment you terminate it and/or exercise your right of cancellation (which will imply the corresponding cancellation from the Program). Your data will be kept only for the purpose of meeting the legal responsibilities that may arise from the services provided for a period of five (5) years.

 How can you exercise your data protection rights?

You can: (i) Send a letter to the Hospital at its main address, or (ii) Send an email to the address of the Hospital, enclosing a photocopy of your identity document, at any time and free of charge, to: (i) Send an email to the address of the Hospital, enclosing a photocopy of your identity document, at any time and free of charge, to:

  • Obtain confirmation as to whether your personal data is being processed at the Hospital.
  • Access to your personal data.
  • Rectify inaccurate or incomplete personal data.
  • Request the deletion of your personal data when, among other reasons, the personal data is no longer necessary for the purposes for which it was collected.
  • Obtain from the Hospital the limitation of the processing of personal data when any of the conditions provided for in the data protection regulations are met.
  • Request the portability of the data provided in those cases provided for in the regulations.
  • To contact the Hospital by e-mail.
  • If you consider that the above rights have not been properly addressed by the Hospital, you may file a complaint with the Spanish Data Protection Agency at the address C/ Jorge Juan, 6, 28001 – Madrid.

In order to exercise your data protection rights, it will be necessary to provide us with your ID card or equivalent supporting document.
